Background: COVID-19 vaccination is still only accepted by half of lactating women in Europe. Further scientific evidence on the safety of the COVID-19 vaccine could help overcome vaccine hesitancy in this population.

Objectives: To describe and compare the incidence of local and systemic solicited adverse drug reactions (ADRs) following the booster dose of different COVID-19 vaccines in lactating women versus the general female population.

Methods:

Design: Web-based prospective observational cohort event monitoring study.
Setting: In nine European countries between February 2021 and November 2022, lactating women, and women from the general population, who received a booster dose of COVID-19 vaccine, were included in the study if they registered within 48 hours after vaccination and completed at least the baseline questionnaire and one follow-up questionnaire.
Exposure: COVID-19 booster vaccine of different brands.
Main outcome measures: The proportion of solicited ADRs following the booster dose by vaccine brand was computed across participating countries in lactating and non-lactating women from the general population.
Statistical analysis: Descriptive analysis of medical characteristics of lactating vaccinees, and a random sample (1:4 ratio) of age- and vaccine brand-matched vaccinees from the general female population. Comparison of the proportion of solicited local and systemic ADRs in lactating and non-lactating vaccinees, following a booster dose of COVID-19 vaccine, and stratified by vaccine brand.

Results: Overall, 124 lactating women were matched with 493 women from the general population. Medical comorbidities were comparable between both groups: 30.6% among lactating women and 37.7% in the general female population.
The vaccine brands used by lactating women were as follows: 52.4% Cominarty (Pfizer/BioNtech), 46.0% Spikevax (Moderna), 0.8% Nuvaxovid (Novavax), and 0.8% unknown brand.
The lactating and non-lactating women reported the occurrence of at least one solicited ADR in a comparable proportion (75.8% and 71.8% respectively). Lactating and non-lactating women reported similar local ADRs (e.g. erythema, hematoma, induration, inflammation, pain) and systemic ADRs (e.g. arthralgia, chills, fatigue, headache, myalgia, and pyrexia). Solicited ADRs were higher after Spikevax (Moderna) than after Comirnaty (Pfizer/BioNtech) booster vaccines for lactating (84.2% vs 69.2%) and non-lactating women (77.2% vs 67.3%), but these differences were not significant.

Conclusions: This prospective study on COVID-19 vaccine safety monitoring showed that lactating vaccinated women and vaccinated women from the general population reported similar solicited ADRs.
