(055) Insulin Consumption Trends in Greece: A Retrospective Analysis of Diabetes consumption based on EOPPY Dataset (2015-2020)

Background: Diabetes mellitus, a chronic and multifactorial disease characterized by high blood sugar levels, affects millions worldwide. Insulin therapy remains central in diabetes management, with evolving pharmacological advancements and consumption patterns. This research addresses the critical need for comprehensive understanding of insulin utilization globally, in the context of diabetes epidemiology and insulin pharmacology.

Objectives: To record and analyze insulin consumption in relation to different diabetes types, assessing use with respect to demographic characteristics and diabetes classifications

Methods: This retrospective study analyzes data from EOPYY-insured populations prescribed insulin for various diabetes types. Data encompasses distinct patient demographics, ICD-10, and ATC level 3, A10A. The ICD-10 diseases range from insulin-dependent to neonatal diabetes mellitus. Data analysis involves descriptive and comparative evaluations of annual insulin prescribing from 2015 to 2020, focusing on age, gender, and disease type correlations. Odds ratios with 95% CI assess the significance of these correlations.

Results: From 2015 to 2020, the insulin consumption were 1,174,425 prescription with ATC3 A10A. The consumption was almost evenly split among genders with men (50.3%) and women (49.7%) amongst all years. Men consistently showed higher consumption rates of insulin in Type 1 Diabetes (E10), ranging from 50.7% to 53.8%, and Malnutrition-Related Diabetes (E12), with rates between 54.2% and 72.0%. The corresponding odds ratios (OR) for E10 varied from 0.941 (95% CI: 0.917-0.966) to 1.118 (95% CI: 1.092-1.144), while for E12, they ranged from 0.727 (95% CI: 0.570-0.927) to 2.415 (95% CI: 1.009-5.783). Women exhibited higher consumption in Diabetes in Pregnancy (O24), ranging from 65.8% to 99.8%, with ORs from 3.003 (95% CI: 2.978-3.045) to 3.764 (95% CI: 3.504-4.046). In age group comparisons, the 0-19 age group had higher usage of insulin in E10, with rates between 2.9% and 8.8% and ORs ranging from 11.955 (95% CI: 10.569-13.523) to 322.406 (95% CI: 239.353-434.279). Conversely, the 60+ age group showed a greater usage in Type 2 Diabetes (E11), with consumption rates from 82.5% to 84.0% and ORs between 2.000 (95% CI: 1.958-2.042) and 7.489 (95% CI: 7.311-7.673).

Conclusions: This analysis highlights the dynamic landscape of insulin therapy in diabetes management. The findings emphasize the need for targeted strategies to improve equitable access to insulin and optimize treatment regimens. Understanding global insulin consumption patterns is crucial for developing effective public health interventions and ensuring optimal diabetes care worldwide
