Background: The incidences of injuries in children and adolescents vary over time according to age, sex, seasons, social environment including location, nutrition status, and health care systems. Analysis of time trends of injuries may identify epidemiologic changes that call for preventive measures.

Objectives: Here we explored the trend and pattern of distal radius fractures (DRF) among Japanese children defined as those with age under 17 years.

Methods: A cross-sectional survey from January 2007 to December 2021 of participants registered to Japan Medical Data Center (JMDC), including approximately 16 million administrative data, was adopted to the study. From the dataset, we included 60,563 children with age under 17 years who sustained a DRF (ICD-10 code S52.5 and S52.6). Prevalence rate per 100000 persons with 95% confidence interval (CI) was calculated by direct standardization against the 2007 Japanese whole population of children with age between 0 to 17 years. A standardized calendar year and sex adjusted risk ratio compared to that of 6-year-olds was calculated for each age category. A structural change year was calculated from linear regression models.

Results: The standardized prevalence rate of DRF per 100000 persons (95%CI) rose after 2007, peaked at a rate of 582 (564–600) in 2015, and was the lowest in 2020 with rates of 420 (411–429). A structural change was observed in 2017 with an increasing trend until 2017 and decreasing trend from 2017. Standardized prevalence rates peaked in 13-years old boys at a rate of 1630 (1568–1694) and in 10-years old girls at a rate of 720 (678–763) of DRFs per 105 persons. Calendar year and sex adjusted risk ratio as compared to 6-year-olds was highest in 12-year-olds [3.7 (3.5–3.9), p< 0.0001].

Conclusions: The rate of DRF in children in Japan 2007 – 2021 shows an increasing trend until 2017 and a decreasing trend from 2017. Our analysis suggests that a structural change may have occurred in 2017. Underlying reasons for this change may include a decreasing trend of exercise intensity among children from 2017-2018, which from January 2020 was further aggravated by the COVID-19 pandemic, and from 2017 an increasing proportion of children with obesity. These results may support the understanding of trends in DRFs in children in Japan and guide future decisions on possible preventive measures.
