DARWIN EU® - Age-specific burden of RSV-related disease in Europe

Background: Severe acute respiratory infection (SARI) caused by respiratory syncytial virus (RSV) is a significant global health concern. Understanding the age-specific burden of RSV is essential for optimizing preventive measures such as vaccination and guiding evidence-based healthcare policies and regulatory decisions.

Objectives: To examine the age-specific RSV disease-related outcomes in five European countries.

Methods: This multinational retrospective cohort study analysed routinely collected health data from six databases between 2013-2022. The databases included primary care databases (Clinical Practice Research Datalink GOLD [CPRD GOLD] in the UK and Sistema d'Informació per al Desenvolupament de la Investigació en Atenció Primària [SIDIAP] in Spain), primary and outpatient specialist care database (IQVIA Disease Analyzer [DA] in Germany), hospital-based databases (Clinical Data Warehouse of Bordeaux University Hospital [CWD Bordeaux] in France and Institut Municipal Assistència Sanitària Information System of Hospital del Mar [IMASIS] in Spain), and an Estonian biobank (Estonian Biobank [EBB]). The study outcomes were RSV-related hospitalizations, intensive care unit admissions, mortality rates, and co-infections.

Results: Among over 52 million individuals in the databases, 44,467 had RSV diagnostic codes and/or positive laboratory test results. The incidence rate of RSV-related hospitalization in the general population was 35.44 (95% confidence interval [CI], 34.96-35.92) per 100,000 person-years (PY), with the highest incidence rates observed in infants ( < 1 year) (2,730.25 per 100,000 PY, 95% CI, 2,681.25-2,779.90), toddlers and preschoolers (1 – 5 years) (153.41 per 100,000 PY, 95% CI, 148.78-158.15), and older adults (≥60 years) (28.09 per 100,000 PY, 95% CI, 27.23-28.97). Additionally, ICU admission rates ranged from 1% to 37% among hospitalized RSV patients. The 30-day RSV mortality rate varied from 11.8 to 18.1 deaths per 1,000 patients, with highest mortality among older adults (50.3-90.2 deaths per 1,000 patients) compared to fewer or no deaths in children. RSV infection was most prevalent in infants (45 – 80%) and older adults (2 – 32%). The three most co-detected respiratory viruses were influenza virus (1.5% to 8.2%), adenovirus (0.8% to 4.2%), and SARS-CoV-2 (0.5% to 2.9%).

Conclusions: RSV infection is highly prevalent in infants and older adults, with a substantial burden of RSV-related hospitalization in under-fives and a high mortality rate in older adults. This study provides insights into the age-specific burden of RSV-related disease, highlighting the need for targeted preventive and control strategies for these high-risk populations.
