(276) Intensity and duration antidepressant exposure in pregnancy and childhood growth trajectories: results from the Norwegian Mother, Father and Child Cohort Study

Background: Little is known about child growth trajectories in children born to mothers with pre-pregnancy depression/anxiety and their associations with the intensity and duration of prenatal antidepressant exposure.

Objectives: To analyze child growth trajectories up to 8 years of age and the association between intensity and duration of prenatal antidepressant exposure and these trajectories.

Methods: We included 4,218 pregnancy-child dyads (singleton, liveborn) with available data on child Body-Mass-Index (BMI), born to women with depression/anxiety prior to pregnancy in the Norwegian Mother, Father and Child Cohort Study (MoBa). Antidepressant exposure groups were modelled using k-means longitudinal method. Trajectories of age- and sex-standardized BMI z-score (zBMI) compared to general Norwegian population were identified using latent class mixed models. Associations between class membership and antidepressant exposure groups were measured using multinomial logistic regression.

Results: Antidepressant exposure groups include: unexposed (82.1%); early discontinuation proximal to pregnancy (5.7%); late discontinuation among stable users (n=5.0%); late discontinuation among short-term users (n=4.7%) and continuers throughout pregnancy (n=2.5%). Five zBMI trajectories were identified: low stable (33.4%), normative until 3 years of age then increasing (6.1%), normative growth (40.7%), medium to normal (29.9%), and high stable (19.8%). We observed no association between these trajectories and antidepressant exposure groups.

Conclusions: Less than half of children born to mothers with pre-pregnancy depression/anxiety had normative growth trajectories while one fourth of them had either stable low or high BMI throughout childhood. We identified a small proportion of children having abnormal growth with BMI accelerated considerably after 3 years of age. Prenatal antidepressant exposure was not associated with abnormal child growth trajectories.
