Background: In Japan, lung cancer has high incidence and mortality among all cancers. The study is to understand the existing status of the treatment modalities, especially for radiotherapy (RT), in lung cancer patients with distant metastasis in Japan.

Objectives: To demonstrate 1) demographic and clinical characteristics, 2) initial treatment episode, 3) RT patterns in patients with newly diagnosed primary lung cancer with distant metastasis in Japan.

Methods: This was a retrospective cohort study using the Japan Medical Data Center (JMDC) database. A cohort of adult patients with at least two diagnosis codes of primary lung cancer (ICD-10-CM: C34) during January 1st, 2015-March 31st, 2023, and initiated lung cancer treatment was specified. Patients with at least one code of distant metastasis (C77.2-C77.9 and C78.4-C79.9) at diagnosis were included in the study. The initial treatment episode was defined as a period with the first single or multiple treatments before a 90-day gap. Patients were followed up until a record of death, or the last enrollment in JMDC.

Results: A total of 1,625 patients were included in the study, with a median follow-up of 14 months. The mean (SD) age of the cohort was 59 (8.9) years old and 71.2% of patients were male. The median (IQR) time from the initial diagnosis to initial treatment episode was 27 (27) days. The median (IQR) duration of initial treatment episode was 8 (13) months. For treatment involved in the initial treatment episode, chemotherapy (67.2%) and RT (59.2%) were the most frequently performed treatment, and surgery was the least preformed (11.7%). In initial treatment episode, single treatment was performed in 707 (43.2%) patients among which 109 (15.5%) patients applied RT only. In multiple treatment, RT with chemotherapy was mostly performed (N=676, 73.2%). For RT patten, the median (IQR) time to the 1st RT was 35 (129) days, and the median (IQR) RT fractions were 22 (27) with a median (IQR) duration of 5 (7.4) weeks.

Conclusions: In Japan, the initial treatment episode usually starts in a month and lasts about 8 months for primary lung cancer with distant metastasis. RT is performed in more than a half of the patients. RT with chemotherapy is the mostly performed treatment in the initial treatment episode.
